Here are the forms for the only required documentation you need to provide to your local school division to comply with the registration and reporting regulations as a home based educator in Saskatchewan!

• Ministry of Education's Notice of Intent and Registration of a Home-based Education Program
  • Be sure to send in this document along with your Written Education Plan each year, either before August 15 or September 15 (the date varies by school division - check with your division for the right date).
    
    
• Written Education Plan - Fillable PDF version  OR  Written Education Plan - MS Word version
  
  
• Annual Progress Report - Fillable PDF version  OR  Annual Progress Report - MS Word version
  • Be sure to submit this toward the end of the academic year before your school division's deadline!
